ABSTRACT

【Objective】 To investigate the efficacy of low intensity pulsed ultrasound (LIPUS) in the treatment of erectile dysfunction (ED). 【Methods】 A total of 121 ED patients treated during June 2020 and June 2022 were selected as the research objects. According to the International Erectile Index Score (IIEF-EF), the patients were divided into three subgroups:mild (17-25 points), moderate (11-16 points), and severe (0-10 points). The total effective rate, erectile hardness scale (EHS), sex life log questions (SEP), general assessment questionnaire (GAQ), peak systolic velocity (PSV), end diastolic velocity (EDV), and adverse reactions of the three groups before treatment, 4 weeks and 12 weeks after treatment were analyzed. 【Results】 A total of 119 patients completed the follow-up. There were significant increases in IIEF-EF and EHS at week 4 and 12 (P<0.05), and the total effective rate was 69.75% and 76.47%, respectively. The total effective rate was significantly higher in the mild and moderate groups than in the severe group at week 4 and 12 (P<0.05). The patients who answered "yes" to SEP2 and SEP3 accounted for 91.60% and 71.43%, respectively at week 4, and 92.44% and 78.15% at week 12, both significantly higher than the rates before treatment (52.10% and 27.73%, P<0.05). The proportion of patients who answered "yes" to GAQl and GAQ2 at week 4 were 84.87% and 71.43%, respectively, and were 82.35% and 70.59% respectively at week 12, with no significant difference. The PSV level significantly increased at week 12 compared to that before treatment [(48.85±14.11) cm/s vs. (41.42±14.90) cm/s] (P<0.05), while the EDV level significantly decreased [(-0.57±7.01) cm/s vs. (2.25± 5.68)cm/s] (P<0.05). 【Conclusion】 LIPUS can improve erectile function in ED patients without obvious adverse reactions.

ABSTRACT

BACKGROUND: Angiotensin converting enzyme (ACE) inhibitor and angiotensin receptor blocker (ARB) may induce acutekidney injury (AKI).The aim of this study was to evaluate the role of the resistive index (RI), which reflects renal artery resistance on renal duplex ultrasonography, as a predictor of AKI in chronic kidney disease (CKD) patients who are prescribed an ACE inhibitor or ARB. METHODS: We screened 105 CKD patients evaluated with renal duplex ultrasonography from 2008 to 2012. We excluded patients not treated with ACE inhibitor or ARB and diagnosed with renal artery stenosis. Finally, we retrospectively analyzed the medical records of 54 patients. AKI was defined as increased serum creatinine by >30% compared with baseline after starting ACE inhibitor or ARB treatment. RESULTS: The mean age of the patients was 60.5+/-13.0 years, serum creatinine level was 1.85+/-0.85 mg/dL and 22.2% of the patients had AKI after the use of an ACE inhibitor or ARB. The RI (P=0.006) and the percentages of patients with diabetes (P=0.008)and using diuretics (P=0.046) were higher in the AKI group.The area under the receiver operating characteristics curve for the prediction of AKI was 0.736 (95% confidence interval=0.587-0.885, P=0.013),and RI> or =0.80 predicted AKI with 83.3% sensitivity and 61.9% specificity. In the multivariate analysis, RI> or =0.80 was an independent prognostic factor [Exp (B)=8.03, 95% confidence interval=1.14-56.74, P=0.037] for AKI. CONCLUSION: RI> or =0.80 on the renal duplex ultrasonography may be a helpful predictor for AKI in CKD patients who are prescribed an ACE inhibitor or ARB.

ABSTRACT

PURPOSE: About 30% of the cause of admission of the patient with end-stage renal disease is reported to be related with complications of vascular access (VA). To achieve good outcome after VA surgery, routine mapping of artery and vein is recommended for the patients who are planned to have vascular access surgery. But evidence supporting routine application of ultrasonogram (USG) is scarce and the situation in Korea is different from western countries where most of the guidelines are produced. We compared the results of the vascular access surgery after physical examination only with those after selective examination with USG. METHODS: Two hundred and forty eight consecutive patients who received VA surgery in Dongsan Medical Center from Jun 2010 to May 2011 were included. Clinical data were retrospectively analyzed and the immediate postoperative failure and early suitability of hemodialysis were compared. RESULTS: One hundred eight patients (group 1) received VA without USG, 140 after USG study. Overall 13 failures were developed. There was no difference between the two groups in terms of early failure (5.6% vs. 5.0%). CONCLUSION: Selective application of duplex sonographic evaluation of vascular status in the patients who need vascular access surgery in Korea is a reasonable policy to save the limited health financial source with acceptable results.

ABSTRACT

Extracranial carotid artery aneurysm is a rare disease. A 72-year-old man was admitted with a painful neck mass for 5 days. Neck CT angiography and carotid duplex ultrasonography revealed a fusiform aneurysm of the left carotid bifurcation with a thrombus. His symptoms improved after anticoagulation therapy. The thrombus decreased in size and ultimately disappeared, as evidenced by serial duplex examinations. Carotid duplex ultrasonography is a simple and useful tool for diagnosis and follow-up.

ABSTRACT

INTRODUÇÃO: A varicocele é a dilatação das veias do plexo pampiniforme do testículo. A associação entre a varicocele e a infertilidade é diagnosticada em 20 a 40% dos homens inférteis. Atualmente, o estudo ultrassonográfico da varicocele é realizado com o paciente na posição supina antes e durante a realização de Valsalva, levando-se em consideração o diâmetro das veias (> 0,20cm), e a presença de refluxo maior que 1, segundo ao estudo Doppler espectral. OBJETIVO: A) Propor uma nova metodologia na avaliação ultrassonográfica da varicocele. B) Avaliar a concordância dos achados no exame ultrassonográfico com Doppler colorido dos vasos dos plexos pampiniformes e as alterações no espermograma na infertilidade masculina. C) Avaliar a correlação entre o diâmetro de um dos vasos dos plexos pampiniformes e as alterações no espermograma na infertilidade masculina. MÉTODOS: Foram examinados 266 pacientes, provenientes do setor de reprodução humana do Hospital das Clínicas da Universidade de São Paulo (HCUSP), por meio de duplex Doppler colorido, inicialmente com o paciente em decúbito dorsal e posteriormente, após 5 minutos de espera, na posição ortostática, realizando as medidas dos diâmetros transversais dos plexos pampiniforme nas duas posições, tanto em repouso como em Valsalva. Ao estudo pulsado foi considerado refluxo patológico apenas quando persistia por mais de 1 segundo, com uma velocidade superior que 2 cm/s. Estes pacientes também foram submetidos a analise seminal. RESULTADOS: Em relação à metodologia de realização do exame, observou-se aumento no diâmetro do plexo pampiniforme apenas com a variação da posição de cerca de 18% à direita e 5,9% à esquerda, bem como aumento na detecção do refluxo venoso na posição ortostática, de 23% à direita e 6,8% à esquerda. Observou-se também correlação direta entre as alterações no espermograma com o refluxo venoso, 65% à direita e 86% à esquerda, o mesmo não ocorrendo com o diâmetro do plexo...


INTRODUCTION: Varicocele is the dilatation of the veins of the pampiniform plexus of the testicle. The association between varicocele and infertility is diagnosed in 20 to 40% of the infertile men. Currently, in the ultrasound study of varicocele, the diameter of the veins is carried through with the patient in the supine position, before and during the Valsalva maneuver, taking the diameter of the veins consideration (> 0,20cm), and a more than 1 second bigger presence of reflux according to spectral Doppler study. OBJECTIVE: A) To consider a new methodology in the ultrasonographic evaluation of varicocele. B) To evaluate the agreement of the findings in the colorful Doppler ultrasonographic examination of the pampiniform plexus vases with the alterations in the spermogram in masculine infertility. C) To evaluate the correlation between the diameter of one the pampiniform plexus vases and the alterations in the spermogram in the masculine infertility. METHODS: 266 patients from the reproduction sector human being of the Hospital of the Clinics of the University of São Paulo (HCUSP) had been examined, by means of colorful duplex-Doppler, initially with the patient in dorsal decubitus and later, after 5 minutes, in the orthostatic position, carrying through the transversal measures of the diameter of the pampiniform plexus in the two positions, in rest and in Valsalva. To the pulse study reflux was considered pathological only when persisted for more than 1 second, with speed superior to 2 cm/s. These patients were also submitted to seminal analysis. RESULTS: Regarding the methodology of accomplishment of the examination, an increase in the diameter of pampiniform plexus was noted only with the variation of the examination position, about 18% to the right and 5,9% to the left, as well as an increase in the detection of venous reflux in the orthostatic position, 23% to right and 6,8% to the left. ABSTRACT

PURPOSE: There is a high prevalence of erectile dysfunction (ED) after electric injuries, but our medical understanding of ED after electric injuries is scanty at best. Thus, the authors attempted to investigate nocturnal penile tumescence (NPT), bulbocavernosus reflex latency (BCRL) and penile duplex Doppler ultrasonography (PDDU) for the patients who suffer from ED after electric injuries. MATERIALS AND METHODS: Of the patients who visited our Burn Care Center between January 2005 and February 2006, ten male patients (aged 20 or older) who complaining of ED after electric injuries underwent NPT, BCRL and PDDU. RESULTS: The patients' mean age was 38.1 years (age range: 25-54), and the numbers of patients exposed to whole body, upper-lower extremity and upper-upper extremity pathways of the electrical current were 1, 2 and 7, respectively. For the patient exposed to the whole body pathway, abnormal findings were observed on all the tests. For one patient who experienced two upper-lower extremity pathways, normal findings were seen on all the tests, while the other patient showed an abnormal NPT, an normal BCRL and an arteriogenic ED. Three of the patients who experienced the upper-upper extremity pathway showed normal findings on all the tests. The other two in the group showed an abnormal NPT, a normal BCRL and arteriogenic ED; the other one showed a normal vascular function, a abnormal NPT and no reaction to BCRL; the other one showed an abnormal NPT, a delayed BCRL and an arteriogenic ED. CONCLUSIONS: More abnormal findings were seen in NPT, BCRL and PDDU for the cases associated with upper-lower body or whole body electrical current pathways, as compared to patients whose electrical pathways were limited to the upper body.

ABSTRACT

PURPOSE: Retrograde flow is the main criterion for the diagnosis of varicocele, and the Duplex Doppler ultrasonographic values for retrograde flows are poorly defined. The purpose of this study is to assess a relationship between the palpated varicocele and pampiniform plexus diameter and the retrograde flow that was investigated with performing Duplex Doppler ultrasonography (DDU). MATERIALS AND METHODS: Physical examinations and DDU were performed to assess varicoceles in forty patients with left-sided varicocele and also in eight men who were without varicocele on their physical examination. During both normal respiration and the Valsalva's maneuver, the maximum diameters of the pampiniform plexus and the presence of the retrograde flow were measured by DDU. RESULTS: The highest mean venous diameter in the patients with clinical grade III varicocele was 2.7mm (2.4-3.6), and this was significantly higher than that in the patients with grade I (1.9mm) or with grade II varicocele (2.1mm) (p<0.05). While the difference of diameters between grade I and grade II was not significant, venous reflux was found in all degrees of the varicocele veins. Venous reflux during the Valsalva maneuver (type II flow pattern) was detected on the right side in one (8%) of 13 patients with grade II varicocele, and in three (21%) of 14 patients with grade III varicocele. The type III flow patterns, indicating augmenting of the reflux with an intra-abdominal pressure increase, were 46.1%, 69.2% and 100% for grade I, II and III, respectively. No venous reflux was found in the control group. CONCLUSIONS: The presence of retrograde flow may provide a more reliable indicator than does the spermatic vein diameter for the diagnosis of varicocele. When the palpability of the pampiniform plexus is questionable, Duplex Doppler ultrasound is indicated.

ABSTRACT

PURPOSE: The purpose of this study was to validate the value of Duplex Color Doppler Ultrasonography (DDU) in post-operative deepvein thrombosis (DVT) after hip replacement surgery. MATERIALS AND METHODS: From September 1998 to June 2000, 138 cases (130 patients) were eligible for a minimal one-year follow-up.DDU was carried out between the 7th and 10th postoperative in all cases, patients wore a compressive stocking on the lower extremity. RESULTS: DVT occurred in five cases (4%), four cases occurred at 10th postoperative day and one case was occurred after one month. DVT occurred in the femoral vein in all cases. Three cases had clinical symptoms such as pain and localized edema. However, no case complained of systemic pulmonary symptoms. CONCLUSION: The incidence of DVT obtained from DDU after replacement hip surgery was very low (4%) with early mobilization and acompressive stocking. No case of DVT progressed to symptomatic pulmonary embolism. DDU was valuable for detecting DVT in the proximal thigh as a screening test.
